Welcome to Bay Area Urology. Communication is a very important aspect in our relationship. To begin our association, we will need more information about you. Please come into the office 15 minutes early to fill out a patient history form. During your routine office work-up, you will be asked to give a urine specimen. Please be prepared to urinate. Do not bring a specimen with you. If you have had lab tests (PSA, urine cultures, etc.) or x-rays results, please bring them with you.

Due to the high cost of billing, we ask that you be prepared to pay at the time of your visit for your services, patient portion, or co-payment depending on your insurance coverage. If your insurance company requires an authorization or referral, it is required prior to being seen. You will need to bring your insurance card. As a courtesy, we do accept Visa and MasterCard. If you should have additional questions, please feel free to call. We look forward to meeting you.

Office Appointments
Both offices are open Monday through Friday from 8:00 a.m. to 4:30 p.m. To schedule an appointment please call the office closest to you. You may wish to call early in order to get a better selection of appointment, days and times. If you need to be seen on an urgent basis, please tell us when you telephone.

If you are unable to keep an appointment, please notify our office as soon as possible.

If you have an emergency after hours, on weekends, or on holidays, you can reach the physician on call by dialing either office telephone number. The answering service will answer your call and will reach either Dr. Karp, Dr. Alver, Dr. Paola, or Dr. Baker.

Please bring all of your medications or a complete list of all your medications, including dose and strength, with you when you come to either the office or hospital.

Emergency Patients
Please call the office closest to you and we will attempt to see you as soon as possible. After office hours, the answering service will answer and contact Dr. Karp, Dr. Alver, Dr. Paola, or Dr. Baker.

If the emergency is life threatening, proceed to the nearest Emergency Room immediately! Ask the hospital staff to contact Dr. Karp, Dr. Alver, Dr. Paola, or Dr. Baker.

Medicare
We accept fee assignment of Medicare claims. These claims will be submitted directly to Medicare by our office. For regular office visits, we request the portion of the fee not paid by Medicare at the time of service.

Insurance
We will file and accept most insurance programs for hospital stays, procedures, diagnostic tests and operations. Please ask us about your particular insurance. Because we are unable to file insurance for routine office visits with most insurance plans, our policy is to accept payment for services at the time they are given. Our staff will be glad to assist you as you file your insurance claims.

Please bring your insurance identification card and claim form with you to your appointment. If your insurance plans requires a referral for your office visit it is your responsibility to obtain the referral from your Primary Care Physician prior to your visit and bring it with you.

Please remember that your insurance is a contract between you and your insurance carrier. While we will be glad to assist in any way possible, the ultimate responsibility for payment rests with you.

Hospitalization
Most of our patients are hospitalized at either Brandon Hospital or South Bay Hospital. However, the doctors are on staff at several other area hospitals in the area.

Dr. Karp, Dr. Alver, Dr. Paola, and Dr. Baker will be checking in on and caring for you on a daily basis should you be hospitalized.

Telephone Calls
All patients are encouraged to call with any questions they have on medical problems or office procedures. However, it would be unfair to our patients for the doctor to interrupt treatment to answer non-emergency phone calls. Our office staff has been trained to answer many of your questions. If they are unable to help you, the staff member who answered your call will relay your information to the doctor and your call will be returned or answered at the earliest opportunity.
